Faculty & Student  Intern Requirements

Nursing & Allied Health Student Clinical Rotations Only

• Active contract and certificate of insurability on file
• Institution can only make a request using the appropriate student request form; no student requests will be honored.
• Complete all health, background and drug screenings
• Complete orientation requirements and submit Affirmation
• Submit Attestation form
• Receive confimation of rotation clearance

• Download and review with all faculty and staff prior to arriving on site - Internship Manual

• All students and faculty members on site must complete the orientation.

• At the conclusion of your orientation, please sign and submit the Student and Faculty Affirmation.

• Complete all health, background a drug screenings
• Complete orientation requirements and submit Affirmation
• Submit Attestation form
• Complete Computer Orientation
• Obtain Bethesda Health ID Badge
• All Bethesda Health ID badges must be returned at the end of each clinical rotation to the ERC. Any unreturned badges will be charged $10 per badge.
• Returning instructors will report to the ERC to obtain their badge for their next clinical rotation.

• Students must wear a school issued ID badge while in the hospital. 
• Faculty will be issued a Bethesda Health ID badge upon receiving clearance and completing computer orientation. 
• ID badges must be visible, above the waist, and worn with the photo side displayed with no object obscuring the photograph or name. 
• During Flu season, Flu stickers must be visible on all badges.

• Students and Faculty, with proof of vaccination, will receive a flu vaccination sticker for their ID badge.
• Students and Faculty declining vaccination must submit declination form and wear a mask at all times while on site.
